Uploading a census

You can enter your group's member/dependent info into an Excel file and import into PerfectQuote to retrieve hundreds of ACA Medical plans, as well as Aetna Alt-Funded Medical plans and fully-underwritten Beam Dental, Vision and Basic Life plans in just a few seconds!

• Go to the "Forms" view to download the PerfectQuote census xls form.
• When opening the file, please enable macros (these are solely to minimize errors in information entered).
• Please review the "Instructions" tab.
• On the "Census" tab, please enter:
  • Group information: Employer name, Zip Code, Effective Date
  • Member and dependent information: last name, gender, date of birth*, relationship, zip code.
    • You can either input ages or date of birth at effective date. Adding dates of birth will automatically calculate the ages.
    • Dependents must be listed directly under the member.
  • Please make sure to follow your state's rules, regarding dependents over the age of 26.
  • Plan selection details, including:
    • Medical Tier – EE, ES, EC, EF, waived coverage, or waiting period.
    • Medical Plan Selection – Plan 1, Plan 2, Plan 3 etc.
    • Add elections for members and dependents in the Dental Tier, Vision Tier, and Basic Life columns.
  • Optional member details if necessary by clicking + at the top of column T.
• Double check for errors
• Save your file and upload to PerfectQuote by clicking “Upload” on the Census tab within a group.

• Once census is uploaded, triple check for errors, and click “Submit Census” to retrieve applicable plans.
